You can download an interpreter claim form here: DOWNLOAD. Remember to complete a new form each time you provide service.

Using a Mobile Device?   You can download the FREE app Adobe Fill and Sign to complete and sign your form.       Select your device:  Android  -or-  iPhone

After you complete your interpretation service and your claim form upload it here: SUBMIT.*

If this is your first time interpreting for InterFaith Works you must FIRST download and complete a IRS W9 Tax Form and a Contract for Services.  then upload them here: SUBMIT*
